AF Civilian ID Form 354 to be discontinued

LITTLE ROCK AIR FORCE BASE, Ark. --  For the past two years, retired Department of Defense civilians have been slowly crossing over from the Air Force Form 354 Civilian Identification Card to a DOD Civilian Retiree Card.

Slated for early spring, the AF Form 354 will be discontinued at Little Rock Air Force Base and only the DOD Civilian Retiree Cards will allow civilians access on the installation and entry to morale, welfare and recreation facilities.

According to the Defense Manpower Data Center, since many retired DOD civilians have no way of identifying their association with the DOD, this card provides them with a trusted credential to establish their identity and affiliation.

Some, but not all, military installations allow retired civilians access to MWR facilities with proper identification. 

Once the AF Form 354 is officially rescinded, installation security will confiscate them when presented for access, so civilians should not wait to replace their access cards.

The installation commander retains the authority to restrict access to MWR facilities for reasons such as local demand, facility capacity and security concerns.

To receive a retiree card at Little Rock AFB, go to the civilian personnel office located on the second floor in building 1255. There you will fill out a form in order to receive the card. Take the form to the security forces office located at building 481, where a card will be created for you.

Unlike the 354 form where some didn't have expiration dates, the ID card needs to be renewed every four years.
