School, sports physicals available to children Published June 22, 2010 LITTLE ROCK AIR FORCE BASE, Ark. -- School and sports physicals are available at the 19th Medical Group for the upcoming school year. If a school requires a special form, people must bring it with them to the appointment. Also, parents or guardians must bring a copy of the child's shot record. In order to get a school or sports physical appointment at the 19th Medical Group, people must be enrolled for care there; those enrolled to an off-base provider must see them for the physical. If someone seen off-base would like to enroll at the medical group to take advantage of this service, visit the Tricare Service Center located in the main medical group building. Officials advise sponsors to plan ahead. If a child will be participating in a sports activity or needs a school physical, it's best to schedule early to avoid a possible delay as appointments are based on provider availability and mission requirements. There are three options for sponsors: Option 1 - Book sports physical online at www.tricare.mil. Be sure to select the "school physical" visit reason. Option 2 - If people can't locate an appointment online, call the appointment line at 987-8811. Option 3 - For additional flexibility, people may also utilize the Jacksonville Clinic for a $52 self-pay. If a child is enrolled to the Jacksonville Clinic the cost is $35.
